Submit A Care Form

Submitting a CARE Form alerts the Office of Student Outreach & Support that a student may need additional assistance: whether academic, personal, emotional, or behavioral. Once submitted, a trained member of the CARE Team will review the concern, reach out to the student, and connect them with appropriate campus resources.

The CARE Team is a cross-divisional group coordinated through the Office of Student Outreach & Support (SoS). The team uses a proactive, collaborative approach to identify and support students who may be struggling with academic, emotional, or behavioral challenges.

Our goal is early intervention — connecting students to the right resources before difficulties become crises.

The CARE Team follows the NaBITA (National Behavioral Intervention Team Association) framework, ensuring a standardized and compassionate process for assessing student needs and coordinating outreach.

When to Use the CARE Form

The CARE Form should be used to report any behavior of concern involving a Fairmont State University student—whether those behaviors occur in or outside the classroom.

Examples of situations that warrant a CARE referral include:

  • Noticeable changes in behavior, mood, or attendance
  • Expression of hopelessness or distress
  • Disruptive or concerning classroom behavior
  • Sudden decline in academic performance
  • Signs of housing, food, or financial insecurity
  • Students returning from hospitalization or extended absence

Always act with an abundance of caution: See Something. Say Something. Do Something.

Be aware that anonymous reporting limits the effectiveness of dealing with concerning behavior.
CARE Forms submitted after normal University business hours will be reviewed as soon as possible the next business day.

Crisis Response Protocol

Determine if the student is a danger to self, or others, or does the student need immediate assistance for any reason?

Yes

The students’ conduct is clearly reckless, disorderly, dangerous, or threatening. This includes self-harm or brandishing a weapon.

IMMEDIATELY: CALL CAMPUS POLICE (304) 367-HELP (4357) OR 9-1-1.

After speaking with campus police or 911, report concern by filling out the CARE form

I'm Not Sure

The student shows signs of distress, but I am not sure how serious this is. After interacting with the student, I feel uneasy and/or concerned about the situation.

During office hours, stay with the student and walk them to the Office of Student Health, if they are willing. Located on the 3rd Floor of the Falcon Center, next to the cafeteria.

If it is after-hours or the student is unwilling to go to the Office of Student Health, fill out a CARE form.

I am not concerned for the students’ immediate safety, but they are having significant academic and/or personal issues and could use support.

Fill out a CARE form which will be triaged by a member of the Office of Student Outreach and Support (SOS).

Who Can Submit a CARE Report

Any member of the Falcon Community—faculty, staff, students, family, or friends—can submit a CARE report if they are concerned about a student’s wellbeing or safety.

While anonymous submissions are accepted, providing your contact information helps the CARE Team better understand the concern and provide timely follow-up.

Note: CARE Forms submitted after normal business hours will be reviewed on the next business day. Immediate threats or emergencies should always be reported to Campus Police (304-367-HELP) or 911.

What Happens After a CARE Form is Submitted

  1. Review & Triage: A member of the Office of Student Outreach & Support reviews the report to determine the level of concern and next steps.
  2. CARE Team Coordination: The CARE Team meets weekly to review active cases, identify trends, and coordinate campus response.
  3. Outreach to Student: A CARE Team representative (typically from SoS) contacts the student, assesses their needs, and offers individualized support.
  4. Follow-Up: Depending on the situation, the student may be referred to resources such as:
    • Counseling or Health Services
    • Accessibility Services
    • Academic Advising
    • Residence Life
    • Financial Aid or Basic Needs Assistance (The Nest)
  5. Ongoing Support: The SoS maintains contact until the concern is resolved or appropriate supports are in place.

All information is handled with discretion and shared only with those who need to know to support the student.
