Institutional & Professional Dues and Fees

Fairmont State participates in associations and/or memberships (institutional, professional or accreditation) that may require annual dues.  In order for Fairmont State to pay these annual dues, they must be listed on the Association Dues List of the Current Expenditure Schedule (see link below)

Any department/individual finding a benefit to Fairmont State for participating in an association and/or membership that requires annual dues, must complete an Association Dues Approval Form for institutional dues and complete the Individual Membership Affidavit Form for individual membership dues.

  • Use the link above to view the current expenditure schedule.  If you do not see the organization listed, please complete the appropriate form(s) below.​
• Association Dues Approval Form 
  • Complete this form ONLY if:
    • New dues – they are not on the current Expenditure Schedule
    • Existing Dues – amount billed is greater than a 10% increase
  • Forms must be signed by the Budget Manager and the President Send a copy of the approved form to accountspayable@fairmontstate.edu. If paying by departmental PCard, a copy of the form must be attached to the receipt.  If paying by check, follow standard purchasing policies for payments made by check.
• Individual Membership Association Due Agreement
  • Complete this form ONLY if
    • New dues – they are not on the current Expenditure Schedule
    • Existing Dues – amount billed is greater than a 10% increase
