Organization Registration Form

NOTE: It is necessary that each club and organization notify the Student Activities Office (318 Colebank Hall) of its status regularly for a number of reasons - not the least of which involved eligibility for the funding request process. This form MUST be completed at the start of EVERY semester!

Please attach a copy of your organization's constitution and by-laws (if applicable). If those items do not exist, please include similar supporting documents (such as the mission/purpose and rules/policies).
