Accessibility Services - Accommodation Letter Request Form

Once the Office of Accessibility Services has verified a student’s disability(ies) with the proper documentation, an accommodation letter may be provided to the student to give to their professors. This letter will explain all the appropriate accommodations for the student. The student may choose to use all, some, or none of the approved accommodations in each class. Accommodation letters can be requested at any time. A student must provide his or her professors with a copy of their accommodation letter each semester in order to receive accommodations. Professors are under no obligation to provide academic accommodations until they receive an accommodation letter. Students who encounter problems regarding accommodations are asked to notify the Office of Accessibility Services immediately.
