The following information explains what to do when an injury/illness occurs involving a student or visitor. A blank injury form has been attached to this email for your future use.

Four Easy Steps:

Step 1. Ask individual if they are in need of medical attention. Help with assistance if needed.

Step 2. The student, supervising adult, or visitor must complete the form if the incident results in injury, property damage, or involves a student on any trip directly related to the institutions.

Step 3. Additional pages can be attached if necessary, including reports from witnesses.

Step 4. Submit the completed form to Facility Safety located at the Physical Plant within 24 hours. (fax 304-367-4656) (email

Common Questions:

  • This form is to be used for both Fairmont State University & Pierpont Community & Technical College students/visitors.
  • This form is to be completed for incidents occurring at all FSU and Pierpont locations, including the main campus, the Caperton Center, RCB Aerospace Center, Merchant Street, Center for Workforce Education. This form is also to be used if an incident occurs on any trip directly related to the institutions.

If the individual does not show signs of harm nor require medical attention this form should still be completed and returned.
