PROCEDURE FOR REPORTING



INJURIES INCURRED ON THE JOB

The following procedures are to be observed when college employees, including student employees, are injured on the job.



1. Secure treatment from the college nurse or other appropriate source.

2. Notify the head of the unit within one work day following the injury providing details such as the date, time, and circumstances.



3. Make arrangements with attending physician to begin the required paperwork if a Workers' compensation claim is made. The injured person may designate someone to do this if he/she is unable to do it. Once the physician and injured employee have completed their sections, the form is to be forwarded to the Human Resources Office along with any other of the following applicable forms. The injured person or designee is to complete the FSC Fairmont State College Injury/Illness Report which will be verified and signed off by the employee's immediate supervisor and forwarded to Human Resources. Note: When an injured employee is off work for more than three (3) days, the employee is to also complete a Workers' Compensation or Sick Leave Benefits Election of Option Form. This form along with the FSC/Injury/Illness Report may be obtained in the Human Resources Office.
