Electronic Format Text Book Request Form

Academic accommodations sometimes include the use of an electronic textbook. Electronic textbooks can be read to a student on portable electronic devices and by computer software such as Kurzweil 3000. Once it is determined that electronic textbooks are an appropriate accommodation, the student must complete the following form to obtain a digital copy of their textbook:

*It is the responsibility of the student to provide the Office of Disability Services with a copy of the receipt that confirms the date the requested textbooks were purchased and the price of the textbooks.
