Nursing School Admissions Checklist

Nursing application must be submitted to Enrollment Services with a $20 application fee (Make check payable to “Fairmont State” by December 1st. If you have applied in previous years and were not admitted, you must re-apply, names are not carried over.

ITEMS LISTED BELOW ARE DUE BY DECEMBER 1

  • Official High School, LPN Program, College Transcripts, and/or GED scores. These must come directly from the high school, LPN Program, college, or testing center to Enrollment Services.
  • ACT/SAT Scores. Scores found on the high school transcript can be used. ACT Math and English scores may be improved by taking COMPASS tests or by retaking the ACT/SAT.
  • TEAS Testing and Scores. Visit the “TEAS FAQ” site for more information.  TEAS test must be completed by December 1st.
  • Attain a score of 18 or better on the English portion of the ACT, or an equivalent COMPASS score, or have successfully completed English 0097 Composition Skills.
  • Attain a score of 19 or better on the Math portion of the ACT, or an equivalent COMPASS score, or have successfully completed Math 0095 Elementary Algebra.
  • All pre-requisites must be met prior to December 1. Fall semester grades are counted and are due as soon as fall semester ends.

Required High School or College Courses to Qualify: 

Biology, Chemistry and Algebra are required courses for admissions into the nursing program.  If you have not taken Chemistry, Biology, and Algebra, we suggest you take the following courses:

Biology

  • Biology 1170 (A&P)

Chemistry

  • Science 1103 (Science That Matters) or
  • Science 1110 (Chemistry Of Life) or
  • Chemistry 1101 (General Chemistry) 

Algebra

  • Math 0095 (Elementary Algebra) or
  • Math 1100 (Intermediate Algebra) or
  • Math 1107 (Fundamental Concepts of Mathematics) or
  • Math 1112 (College Algebra)
     

Additionally:

  • You must have at least a 2.0 cumulative grade point average.
  • If you are an LPN, or have been enrolled in an LPN Program, you must submit your transcript by December 1st. If you are licensed, please include a copy of your license.
  • Any transcripts, ACT/SAT scores, TEAS scores, or pre-requisites not received by the end of fall semester will not be considered.
  • If you have changed your name since high school or college, make sure the admissions office has a record of previous name(s).
