Application for Admission - Associate Degree Nursing

The Associate Degree Nursing program is a two-year limited enrollment program.  Graduates of the program are eligible to take the licensing examination to be a Registered Nurse after approval by the West Virginia Board of Examiners for Registered Nurses.  The applicant must submit to the Enrollment Center evidence of all post-secondary education attempted or completed.  Fairmont State must have official high school, college, and LPN transcripts to verify graduates for licensure. ACT, SAT, or COMPASS scores are evaluated in the admissions process.  A nursing pre-test (TEAS) must be completed with a score of 50 or above on each of the 4 components. In order to facilitate processing of your application, please check with the Enrollment Center to make sure they have received these documents.   The application deadline is December 1st for the class beginning in August.  A non refundable application fee of $20 must be paid in the Enrollment Center.  Make check payable to Fairmont State.  Admission to the Nursing Program will be contingent upon your score on the Score Sheet for Selection, the results of a criminal background investigation, and your health status.

(3) Chemistry, Biology, and Algebra are required before admission and must be completed by the application deadline.  These can be high school or college level courses.  High school courses require 2 semesters or 1 credit.  College courses can be one semester.  Have you taken and received a passing grade for chemistry, biology, and algebra either in high school or college? ________

(4) You must have a minimum ACT Math score of 19, or  minimum SAT score of 460, or an equivalent COMPASS score, or have passed Math 0095.  Have you met the minimum Math ACT score, SAT score, Compass score, or taken and passed Math 0095? _____

(5) You must have a minimum ACT English score of 18, or minimum SAT score of 450, or an equivalent COMPASS score, or have passed English 0097 or English 1104. Have you met the minimum English ACT, SAT, or Compass score, or passed English 0097 or English 1104?_______
